I (We) agree that the use of any FNCB ATM/Check Card ("Card") issued in response to this application will constitute my (our) agreement to be jointly and severally bound by the terms and conditions of the FNCB ATM/Check Card Agreement. It is certified that the above information is complete and true, and is given to induce you to issue said Card(s). I (We) authorize First National Community Bank to make whatever credit and/or investigative inquires deemed necessary in connection with this request including but not limited to obtaining a credit report from a consumer reporting agency and acknowledge that this request may be denied based on information obtained in the aforementioned report. A PIN number will be assigned to me in order to access this card. The PIN number will be mailed directly to the address provided. I (We) understand that this Card is not a Credit Card, and that no commitment to extend credit to me (us) will be made by your issuance of the FNCB ATM/Check Card(s) requested. I (We) acknowledge and agree to the terms and conditions as outlined in the FNCB ATM/Check Card agtreement.
